Your lunch menu should consist of at least one, two-course meal options including an entrée with an appetizer or a dessert at a set price point between $10 - $15. Menu choices must be submitted on this form.
Your dinner menu should consist of at least one, three-course meal options at a set price point between $20 - $25. Menu choices must be submitted on this form.
